I authorize Giannini's Auto Body to disassemble, inspect and analyze the damage to my vehicle in order to determine the appropriate method of repair, and estimate the necessary cost to repair the vehicle. I understand that the shop will reassemble my vehicle to the best of their ability.
I authorize Giannini's Auto Body, its employees, and any third party providers to operate my vehicle for the purpose of repairs and test driving to ensure the proper function of my vehicle.
I further acknowledge that if I choose not to have the recommended repairs performed, I will be responsible for a fee of $165/hr with two hour minimum for the disassembly, damage analysis and estimate preparation. I also acknowledge that if I choose not to have the recommended repairs performed I will be responsible for accrued storage in the amount of $350/day beginning three business days after the insurance company or Giannini's Auto Body determines the vehicle to be a total loss, or I decide not to authorize repairs (excluding days of disassembly).
